We are currently experiencing a high volume of referrals and have implemented a waiting list. We do not expect the wait to be long for an initial assessment (currently 2-8 weeks based on clinical urgency and service required).
Please fill out the referral form with as much information as possible to assist us in processing it promptly.
We will contact you to acknowledge receipt of your referral. When booking appointments, please ensure you respond promptly, to avoid the available appointment being allocated to the next person on the waiting list.

You will need to fill in all sections with red asterisks & ensure the phone number you add is in the correct format (no spaces or brackets for area codes).
This form is best completed on a tablet or computer. It can be completed on your phone, but you will need to ensure you scroll down on both the white middle section and grey outer section to get to the bottom of the form.
